Enrolling in a Medicare Chronic Special Needs Plan (C-SNP)

Enrolling in a Medicare Chronic Special Needs Plan (C-SNP) is a process that matches you with a Medicare Advantage plan designed for people with specific severe or disabling chronic conditions.

Who can enroll
You must:

  • Have Medicare Part A and Part B coverage.

  • Live in the plan’s service area.

  • Have a qualifying chronic condition listed by the Centers for Medicare & Medicaid Services (CMS), such as diabetes, chronic heart failure, cardiovascular disease, end-stage renal disease, or other medically complex conditions.

  • Have your doctor verify your condition within 60 days of your first day of coverage to remain enrolled.

How the enrollment process works

  1. Check plan availability – C-SNPs are offered in certain service areas; not all plans are available everywhere.

  2. Select a plan – Compare coverage, benefits, and provider networks. Many C-SNPs include extra services like $0 copays for preventive care, dental/vision/hearing exams, diabetic supplies, and insulin.

  3. Enroll through Medicare – You can enroll during Medicare’s Annual Enrollment Period (AEP) or during the Special Enrollment Period (SEP) if you qualify.

  4. Provide medical verification – Your doctor must confirm your qualifying condition before you can remain in the plan.

  5. Sign up and confirm – Complete the enrollment form, pay any applicable premiums, and confirm your plan details with Medicare.

Key benefits of a C-SNP

  • All benefits of Original Medicare (Part A & B) plus prescription drug coverage.

  • Extra care management and low specialist copays.

  • Additional benefits like preventive care, dental/vision/hearing coverage, and diabetic supplies.

Tip – If you have multiple chronic conditions, a C-SNP may offer more tailored care and lower out-of-pocket costs than a standard Medicare Advantage plan. Always confirm your plan’s availability in your service area before enrolling.
